I used Technical Support. I see that others with the same issue have also posted in Device Functionality, Plans & Services, and Account Services.

Reason: I don't really want to share my real account name in the forum. After logging in to the forum, you can go to preferences and set up a forum-specific alias name before posting.
Finally got both lines restored but 100-minute international calling is gone... Now I have to see if anything else is missing.

I've seen someone elsewhere report the same thing-- and that customer service said they couldn't restore it since it's no longer active. I had it on at least one line, but haven't remembered to look it it's still there.
